We use a HIPPA compliant program to collect and store consent forms. Your consent form must be completed prior to arriving at your appointment. This form will take 4-5 minutes to complete and can be filled out on your smart phone or other device. Having your forms filled out ahead of time cuts down on your wait time in the clinic and helps us care for you more efficiently! This form only needs to be filled out once and will be kept on record for your following appointments.

Our goal is to provide quality preventive dental care to all our clients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our providers, but our other clients as well. We kindly ask that you cancel your appointment at least 24 hours in advanced. Failure to show and cancelling an appointment with less that 24 hours notice will result in a $50 fee (unless related to sudden illness or COVID-19 like symptoms).
Do you have benefits? That's amazing! Many companies (like Canada Life, Medavie Blue Cross, Johnson, Green Shield and more) have enabled us to send electronic claims. This means we can see what your co-pay is in real time and we get reimbursed in a timely manner.
Unfortunately, there are a handful of companies that have not enabled direct billing for dental hygienists. We ask that if you have coverage through Manulife or Sunlife (or another company that doesn't offer direct billing) that you pay up front and get reimbursed directly from your insurance provider. We'll even send in your paper work so that you don't have to do anything with it!
